We are a Catholic mission-oriented health care network serving the communities of the Fox River Valley and east central region of Wisconsin. As an integrated delivery system, we successfully partner physicians, long term care, hospital and insurance services to provide a complete continuum of compassionate care to the people we serve.

Affinity Health System is jointly sponsored by Ministry health Care, Milwaukee, and the Wheaton Franciscan of Wheaton, Illinois. The sisters' tradition of caring for people in our region began a century ago. At Affinity, we are committed to celebrating this rich heritage and continuing our mission of caring for the health and well being of the communities we serve, especially the poor.

Affinity Health System Organizational Structure:

Formerly La Salle Clinic, Affinity Medical Group's roots date back to 1938. The group has grown to include 17 clinics in 12 locations. It is now the largest multispeciality group practice in the region, with nearly 200 physicians and other medical providers, and is the fifth largest in Wisconsin.

Calumet Medical Center offers medical/surgical services, cardiac rehabilitation, ER/ambulance services, long-term care in the swing-bed program, ICU, physical and occupational therapy, child care, radiology, respiratory therapy, home care/hospice, laboratory, sports medicine services and various community outreach programs. The hospital is a 53-bed facility with 14 active physicians on staff.

A 215-bed facility with long-term care, transitional rehabilitation, hospice services and therapy programs. Franciscan Care and Rehabilitation Center offers fully licensed health services in a home-like setting.

Mercy Medical Center combines over a century of service with the latest in technology. Mercy Medical Center offers general medical and surgical services plus specialized services including emergency, obstetrics, cardiac and intensive care.

Connected to the new Mercy Medical Center, Mercy Oakwood is an integral partner in providing outpatient care. Mercy Oakwood is a modern facility complementing this state-of-the-art medical campus, with medical offices, outpatient rehabilitation and The Breast Center.

Occupational Health Systems of Wisconsin (OHS) is setting a new standard for occupational healthcare throughout the Fox River Valley. Our clinical specialists are the most experienced providers of continuous and emergency care for workers in the region. The OHS list of services is the most comprehensive menu available in northeast and east central Wisconsin.

St. Elizabeth Hospital

St. Elizabeth's is a 146-bed acute care hospital providing general medical and surgical services and specialized services (emergency, obstetrics, oncology, cardiac, hyperbaric oxygen therapy, subacute and intensive care).

The National Committee for Quality Assurance (NCQA)

Network Health plan is proud to be one of the health plans in the state of Wisconsin to receive commendable accreditation for our HMO product from the National Committee for Quality Assurance (NCQA). The NCQA is a private, non-profit organization that assesses and reports on the quality of managed care plans in the U.S. to help consumers and employers make informed decisions when choosing a health insurance provider.

Joint Commission on Accreditation of Healthcare Organizations (JCAHO)

Affinity Health System is accredited by the JCAHO, an independent, non-profit organization which recognizes hospitals and health care systems for compliance with national standards of quality and performance. Affinity Health System voluntarily undergoes a survey by JCAHO every three years to ensure that it is meeting and exceeding the standards set for patient care.

Wheaton Franciscan Sisters

Wheaton Franciscan Sisters is the sponsoring community for more than 100 health and shelter organizations committed to human and community development.

These organizations are located in four states: Wisconsin, Illinois, Iowa and Colorado. The Sister's facilities in Wisconsin collectively comprise the largest health care provider system in the state, including Covenant Health Care in Milwaukee.

The Sisters continue to govern these services through participation in the WFSI Board and through local boards of directors for the respective health care systems or organizations.

The Wheaton Franciscan Sisters have been providing health care and spiritual aid to Fox Valley residents for more than 100 years and established St. Elizabeth Hospital in November, 1899.

Ministry Health Care

Ministry Health Care, sponsored by the Sisters of the Sorrowful Mother, is a regional health care system founded in 1984 to assure the viability and further development of the Sisters' ministry.

It is the tenth largest among Catholic health care systems in the United States, with nine hospitals (1,364 acute care beds), long-term and assisted living facilities, home health agencies and community centers in Wisconsin and Minnesota. Its facilities include St. Joseph's Hospital in Marshfield and St. Micheal's Hospital in Steven's Point.

The Sisters of the Sorrowful Mother has been providing health Care and spiritual aid to Oshkosh area residents for more than 100 years through Mercy Medical Center, founded in 1891.

Affinity Health System is a dynamic health care organization located in the heart of the Midwest. Affinity Medical Group has clinics located in Clintonville, New London, Appleton, Winneconne, Kaukauna, Menasha, Little Chute, Neenah, Chilton, Oshkosh, Kiel and Ripon.

Affinity's Continuum of Care

Affinity Health System's objective is to create a seamless continuum of care for patients as they move from outpatient or ancillary services to inpatient or home health services.
