Job Description:
Accountable for coordinating, facilitating, monitoring, and evaluating patients' readiness for long term acute services and ensures quality and cost-effective use of clinical resources in a manner consistent with GHS mission and values. Coordination of services includes, but is not limited to: patient/ family education, discharge planning and financial management as related to the PPS and other systems. Accomplishes job by integrating and maximizing the work of various disciplines, resources and services across departmental lines. Promotes patient and family centered care.
Qualifications:
Bachelor's Degree in Nursing with 2 years Critical Care and/or Med-Surg Nursing experience and current SC RN License required.
Minimum Salary:
$21.70/Hour
