Description:

Join our team and live the dream of giving the compassionate care to your patients they so deserve! RN Case Managers (FT) work as part of a team assigned to a portion of Marion County. Weekend and PRN opportunities are available in our Residential Facilities and Field Teams. Duties include: Coordinates, monitors, and follows plan of care for assigned patients; Provides nursing care, education and support to the patient/family and/or care givers; Participates in interdisciplinary team with Social Workers, Counselors, Chaplains, CNA/HHAs, Massage Therapists, Physicians, and team management.
Qualifications:
Current Professional RN License issued by the Florida Board of Nursing; Two years of Med/Surg, Oncology, or Hospice nursing experience; Current CPR; Valid Florida Driver's License, reliable transportation and current auto insurance coverage; Spanish speaking a plus!
Hiring Range:
$20-$24/hr; PRN rate $24/hr
