Social Worker--Full-time-- Day shift

Job Description
HR Anew is a human resources consulting and professional services firm located in Columbia, MD. We help employers recruit, educate, and retain employees. We have been engaged by a 183-bed long-term care facility, to identify the best available Social Worker for full time employment for their District of Columbia location.
Position Summary:
The primary duties and responsibilities for this position include, but are not limited to the following:
- Actively assist with the initial adjustment of new residents while offering supportive services to their families;
- Responsible for case management and evaluating the success of the resident’s placement;
- Complete required documentation according to established policies and procedures, to include MDS, social history, status reports, and progress notes;
- Actively participate as a member of the Inter-Disciplinary Resident Care Conferences;
- Assist residents with financial, medical, and social matters.
- Responsible for one resident support group, as assigned by the Director of Social Services, and assist with activities of the Residents’ Council.
Qualifications:
- Must possess the ability to work caringly with long term care and geriatric patients; communicate with staff, residents, and family members; and document specific information regarding patient care as required;
- B.S. degree in Social Work;
- 2 years experience in long-term care or gerontology;
- Possess valid DC Licensed Social Work Associate minimum;
- Must be able to lift at least 50 lbs;
- Must be legally eligible to work in the United States of America;
- Must be able to successfully complete a reference check and background investigation.
