Health Coach
Job Location: Lafayette, IN
Summary:
Provide telephonic coaching, counseling, consultation and information to patients
Participate with ongoing development of the wellness/lifestyles program.
Maintain accurate and timely documentation to facilitate accurate reporting information for top management.
Research and develop educational offerings to continue the professional, high quality of the practice of coaching.
Communicate with the condition manager when pre-utilization of services and follow-up are needed.
Assess participant needs for wellness program
Assist participants in forming solutions for presenting problem and determine realistic goals
Provide client interventions using plan of care
Intervene effectively with participants who are in crisis
Determine and provide relevant community and/or healthcare resources
Promote wellness and provide education regarding preventive measures and wellness programs
Track data and information
Communicate as needed, with service delivery partners, physicians, and other healthcare professionals.
Attend health fairs and client meetings as needed
