Job Summary:
The Patient Navigator will establish contact with cancer patients within the hospital setting through approaches agreed upon with the host site to raise awareness about American Cancer Society information resources, support programs and services, and assist in meeting other cancer-related needs articulated by patients and families. The Patient Navigator will be responsible for establishing an ongoing relationship with the health care community and the host location of the ACS Patient Navigator Program. The Patient Navigator will be responsible for ongoing evaluation and quality improvement, procedures, recruiting, interviewing, training and supervising volunteers of the American Cancer Society, and helping to enhance and maintain the Community Resources Database. Some travel may be required. Responsible for meeting the goals and objectives established in your individual Performance Achievement Communication Tool (P.A.C.T.)

Essential Duties and Responsibilities:
  • Patient Navigation: Makes cancer patients and families aware of ACS information resources, support programs and services. Provides additional information about community resources for needs articulated by cancer patients and their families who visit the ACS Patient Navigator Program. Conducts follow up calls to patients and families to ensure that the resources met their needs and identify additional needs. Identifies gaps in resources and works within the community to bridge those gaps. Maintains patient confidentiality in accordance with HIPAA regulations. Refrains from formal assessment of patient or family psychosocial needs, counseling, or provision of any type of professional services
  • Project and Meeting Management: Responsible for ongoing recruitment, support and supervision of volunteer navigators. Oversight of shadow/individual training for volunteer navigators. Participates in designated meetings at assigned health care systems, representing EA Division, Regional Staff Meetings and Centralized PAFS/Patient Navigation Meetings, and select community meetings as designated by PAFS Director/ RVP.
  • Data Management: Contributes to the maintenance of the Community Resources Database. Maintains accurate, confidential records documenting services provided and unmet needs.
  • Communication: Works within the host facility to promote Navigation and build patient referrals. Works cooperatively as part of a team with the ACS regional, Division and National offices. Acts as a representative of the American Cancer Society within the entire community and especially within the host community.
  • Administration: Liaison between volunteers and Regional staff at designated health care systems. Collaborates with patient navigator volunteers and health care systems’ staff, including but not limited to Director of Volunteers, Director of Nursing and other PNP champions at health systems.

Nature of Supervision:
Report to: Director, Patient and Family Services

Contacts and Relationships:
Must work closely with Regional Director of PAFS, Regional Vice President, Director of Health Systems and Collaborations for Northern NJ, Director and Patient Service Specialists at the Patient Service Center, and other regional and field staff working to reach EA Division 2010 QoL Outcomes.

Works closely with all EA Division constituents associated with PNP and specified health systems.

*Position available based on grant funding through Astra Zeneca.

Qualifications:
  • Bachelors Degree in a health-related field preferred.
  • Thorough knowledge of health care systems
  • Knowledgeable about the breadth and accessibility of community resources.
  • Medical terminology
  • Working knowledge of HIPAA regulations

Special Work Requirements:
  • Travel and evening and/or weekend work required.
