Job Summary:
The Coordinator Acute Care is the liaison between nursing, the patient care team and the medical staff addressing patient priorities from admission to discharge. The Coordinator Acute Care duties are clinical, quality and documentation as it relates to appropriate coding. It also includes administrative duties to help maximize patient time for the Hospitalists. The coordinator demonstrates behaviors that role model customer service, organizational values and teamwork while assessing achievement of desired clinical and financial outcomes. The coordinator is responsible for communication and coordination of patient care with members of the Personalized Care Delivery Model.
Job Requirements:
RN license required. Completion of BSN within 3 years. Masters prepared RN preferred. CPR certification within 90 days. 1 – 3 years experience in acute care as a Staff RN, preferably in a medical/surgical environment.
