Job Summary:
Performs disease and case management functions for assigned cases (including complex) according to department policies and procedures. Identifies high risk and high cost disease management program patients with complex medical and/or psychosocial needs, assesses options and opportunities for intervention and designs plan to improve quality and efficacy of care. integrates and coordinates the expertise and support of other professionals, family members, community agencies and providers across the health care continuum. Achieves optimal clinical and quality outcomes by effectively managing care and resources based upon evidence based guidelines and approved measures.
Job Requirements:
Graduate of accredited school of professional nursing. BSN preferred. Current license as an RN in the State of WIsconsin. Minimum of 4 years clinical health care experience as an RN--previous Behavioral Health experience is required, additional med/surg experience is preferred. Prior disease management experience preferred. Skilled in basic personal computer applications. Excellent organizational skills to include balancing of multiple projects at one time. Knowledge on disease management and behavior change theory.
