Provides in-community services and crisis intervention to consumers' homes, residence, or other commonly based locations. Provides input on the development of psycho-social assessment, referral to external services, and reviews all rights and responsibilities. Implements treatment plans and collaborates with relevant staff in the development of treatment plans and service provision. Must be able to ensure all clinical and administrative documentation with internal and external agencies are completed in a timely manner.
Requirements: BSW in social work or BA/BS in psychology or a related. Must be able to travel to designated locations in the community to meet with consumers.
