Minimum BS in Physical Therapy. MN license with obtainable WI license. CPR certified. Valid driver's license and current auto insurance. One year physical therapy experience in health care related field.
Responsibilities:
To provide quality care to client in their homes. The therapist addresses risk, promotes precaution education, health and wellness
