Job Description:
Works under the supervision of the Manager-HIS Clerical assisting with the completion, maintenance and release of information comprising patient medical records.

Performs other duties as assigned.

TECHNICALCOMPETENCIES
Employee in this position perform the following technical competencies:
  • Constructs medical records, placing health information documentation in the medical record in correct order and sequence.
  • Analyzes medical records for completion following medical staff bylaws, JCAHO, CMS and DOH guidelines.
  • Routes medical records and advance directives as needed throughout the facility.
  • Retrieves hard copy and microfilm medical records for patient care and review and routes appropriately.

  • Releases information to internal/external customers and is knowledgeable to Federal and State
  • regulations, assisting customers with completion of written requests. Prepares billing for external customers and monitors monthly. Prepares cash deposit weekly.
  • Acts as a receptionist and customer service representative. Answers the phone and makes necessary calls to meet and exceed the needs of the customers.
  • Checks in all deliveries and distributes supplies appropriately.
  • Completes data entry and updates in computer software programs. Retrieves clinical information from HannaH and microfilm as needed.
  • Sorts and routes loose sheets to medical records, placing in appropriate chart order and sequence.
  • Performs other duties as assigned.
  • Assists physicians with chart completion. Faxes info to physicians' offices as requested.
  • Performs case merge (merges multiple medical record numbers). Investigates and merges in multiple systems.
  • Listens to dictation on listen feature to identify dictation and assigns to transcriptionist. Reviews Hannah for signatures and reports.
  • Monitors potential delinquent status of charts weekly. Notifies and follows up with physicians accordingly.
  • Provides input for process improvements.
  • Acts as mentor to newly hired staff.
  • Performs notary duties as applicable.
  • Provides assistance with escorting new admission patients to destination.
  • Retrieves pending reports and places in medical record in appropriate chart order.

Job Qualifications:

EDUCATION:
High School Diploma or GED.

LICENSURE:
None. May be required to obtain notary.

EXPERIENCE:
Basic computer and data entry skills required. Previous work experience in a Health Information Services department desirable.

ESSENTIAL TECHNICAL/MOTOR SKILLS:
Inputting data, speaking clearly, calibrating equipment, answering phone, precise hand/eye coordination, fine motor skills, good writing skills.

INTERPERSONAL SKILLS:
Must be courteous; work in a professional, caring manner with internal and external customers; have the ability to work with interruptions, and flexibility in hours and workflow, foster teamwork and promote service and quality in everything.

ESSENTIAL PHYSICAL REQUIREMENTS:
Assembling, kneeling, crouching, stooping, reaching, standing, walking, pushing, pulling, lifting, operating, lifting 10 lbs.

ESSENTIAL MENTAL ABILITIES:
Interpret data, organizational skills, comprehend. Follow rules/instructions. Adaptability, short and long-term memory, concentration/alertness, reading.

ESSENTIAL SENSORY REQUIREMENTS:
Visual skills, hearing, feeling, distinguish colors, manual dexterity.
