Job Description:
The Patient Care Associate (Hospice) works under the supervision of the Team Leader, Resource Nurse and therapist. Accepts assignment in homes of adult or pediatric patients.

Performs other duties as assigned.

TECHNICALCOMPETENCIES
Employee in this position perform the following technical competencies:

  • Provides personal care as instructed by RN (care includes bathing, dressing changes, nail care, skin care, shampoos and light housekeeping)
  • Transfers patients as instructed by RN and therapist.
  • Documents care provided. Observes and reports changes in patient's condition.
  • Travels to patient homes.
  • Attends patient conferences and staff meetings.
  • Other activities/duties as assigned to support Hospice Department.
  • Accepts assignment from general caseload.
  • Adheres to and supports compliance standards as set forth by Medicare, State and JCAHO.
Job Qualifications:

EDUCATION:
High School education or equivalent. Certified Hospice Aide as defined by competency testing.

LICENSURE CERTIFICATION:
If operating a motor vehicle for business purposes must have a valid driver's license. BLS certification is required. Must be registered and successfully pass background check with the Family Care Safety Registry.

EXPERIENCE:
Hospital/Nursing Home experience or previous Home Care experience.

ESSENTIAL TECHNICAL/MOTOR SKILLS:
Fine motor skills required. Ability to speak clearly. Documents all care provided

INTERPERSONAL SKILLS:
Ability to interact with the elderly is mandatory.

ESSENTIAL PHYSICAL REQUIREMENTS:
Moving, standing, walking, balancing, kneeling, crouching, stooping, reaching, pushing, pulling, lifting, restraining - all involved with direct patient care. Able to transfer patient with appropriate body mechanics, as instructed by RN/Therapist. Able to operate a motor vehicle.

ESSENTIAL MENTAL ABILITIES:
Understands normal patient condition and disease process in order to report changes to RN. Assesses and evaluates patient change in condition and decides if RN needs to be notified. Follows rules and instructions to get to patient homes and to follow accepted procedure in providing personal care.

ESSENTIAL SENSORY REQUIREMENTS:
Visual, feeling, and hearing skills.

EXPOSURE TO HAZARDS:
Road and weather hazards. May be exposed to blood and body fluids.

OTHER:
Must have access to transportation, which will allow for timely scheduled home visits. Be competent to support adult and pediatric home care and to continue education to maintain skills and further knowledge.
