Job Description:
The LPN-IV Certified works under the combined supervision of the Team Leader and Patient Care Coordinator to provide patient care to Hospice patients according to established policies and procedures. Follows the directions of the RN.
Performs other duties as assigned.

TECHNICAL COMPETENCIES
Employees in this position perform the following technical competencies:
Implements care plan for patient and family significant others, assessing patient's conditions. Reports observations of patient's condition to RN.
Provides direct nursing care to patients under direction of primary physician and RN Case Manage, including reading and interpreting physician's order, administration of medications, managing IV's and practicing universal precautions for infection control.
Communicates patient and family needs in a clear and timely manner to appropriate IDT members.
Maintains patient's medications, including receiving physician orders and ordering needed medications.
Teaches family members significant others patient care skills including: pain control, medication administration, dressings, hygiene, nutrition, checking vital signs in accordance with orders from physician and practicing universal precautions for infection control.
Assists patient with activities of daily living and personal needs: bathing, ambulating, transfers, range of motion exercises, elimination needs, eating and self-administered medications.
Provides support to patient's family significant others at the time of patient's death.
Charts all patient care activities according to established agency policies and procedures.
Practices safety measures with durable medical equipment.
Provides on call coverage after hours.
Job Qualifications:
EDUCATION: Graduate of an accredited LPN school.
LICENSURECERTIFICATION: Licensed as a LPN, IV Certified in Missouri and able to obtain license in Kansas if needed. If operating a motor vehicle for business purposes must have a valid driver's license. BLS certification required. Must be registered and successfully pass background check with the Family Care Safety Registry.
EXPERIENCE: Two years working as a LPN in a hospital, skilled or intermediate care facility preferred. Previous experience with dying patients preferred.
ESSENTIAL TECHNICAL/MOTOR SKILLS: Ability to give injections, start IV's, administer medications, change dressings; requires use of fine motor skills, precise hand-eye coordination.
INTERPERSONAL SKILLS: Ability to effectively interact with patients, families significant others, physicians or co-workers.
ESSENTIAL PHYSICAL REQUIREMENTS: Moving, standing, walking, balancing, kneeling, crouching, stooping, reaching, pushing, pulling, lifting, restraining - all involved with direct patient care.
ESSENTIAL MENTAL ABILITIES: Ability to complete a thorough assessment of patient's condition and evaluate effects of care; must be able to visually assess resident's condition.
ESSENTIAL SENSORY REQUIREMENTS: Visual and hearing, touch and smell needed.
EXPOSURE TO HAZARDS: Exposure to needle puncture wounds, cuts; possibility of back injuries while lifting and moving patients; exposure to blood and blood byproducts; exposure to household cleaning chemicals.
OTHER: Must have access to transportation, which will allow for timely scheduled home visits.
