Description:
This position is responsible for growing key service line areas as defined by management. As a critical member of the marketing and admission teams, the Hospital Liaison works closely with hospital discharge planners, physicians, palliative team members, social workers, case managers, management, inpatient unit and education staff to generates referrals. Develops and maintains new and current relationships with assigned hospital(s). The Hospital Liaison also provides initial and on-going assessments of patients in facility contracted beds, and manages patients admitted to the hospital for symptom management.
Qualifications:
Requires RN license, CPR certification and 2 years nursing experience, as well as a valid OH DL and auto insurance.
