Description
We are looking for RN's that have a strong sense of initiative, compassion and drive to be responsible for managing plans of care for patients and families receiving hospice care. This is a rewarding opportunity to meet, work with, stimulate and teach a variety of people with different needs at the end of life.
Care Partners RN Case Managers work with patients and families in nursing homes and extended-care facilities to provide top-of-the line hospice care. This position will cover our South territories from Springboro to West Chester.
Position Requirements
Registered Nurse (RN)-State of Ohio
CPR Certification
Certified Hospice and Palliative Nurse (CHPN) preferred
Active Drivers License
Valid Automobile Insurance
** A Safe driving record is mandatory: no more than 3 moving violations or more than one chargeable accident in the past 36 months and no major convictions (drunk, drug or reckless driving) within the past 7 years.
