Hospice of Havasu
Admitting Nurse
Job Location: Lake Havasu City, AZ
Organizational Relationship:
Reports directly to Admissions Manager
Summary of Function:
Responsible for admission of patients to Hospice of Havasu program.
Make routine home visits, as necessary.
Major Duties and Responsibilities:
1. Following current procedure, plan for admission and/or admit patients to hospice services.
2. Assess initial needs of patient/family.
3. Develop initial patient Plan of Care.
4. Accurately complete documentation required by regulation and agency policy.
5. Provide current information regarding Hospice of Havasu services to patients, physicians and others.
6. Attend IDT, as required.
7. Attend in-services, as required.
8. Make routine home visits, as requested.
9. Work with and effectively communicate with other team members.
10. Other duties as assigned by supervisor.
Requirements:
1. Is a graduate of an approved school of nursing.
2. Is licensed as a registered nurse in the State of Arizona.
3. CPR certification.
4. Able to periodically give total patient care.
5. Able to work independently with little supervision.
6. Demonstrated ability to relate effectively with patients, physicians, community institutions, etc.
