Description: WORK SCHEDULE: WEEK ONE: Monday, Tuesday, Saturday and Sunday 11:15 am to 9:15 pm.
WEEK TWO: Monday, Tuesday and Wednesday 11:15 am to 9:15 pm.
MINIMUM QUALIFICATIONS:
High School Diploma or Equivalent. Assist and support people with disabilities. Applicants must be physically fit. The employee must have the ability to lift / move at least 50 pounds.
SERVICE PLANNING AND IMPLEMENTATION:
Participate as a member of the individual`s interdisciplinary team. Conduct assessments and evaluations. Write training programs which identify short term and long term goals, objectives and/or needs. Responsible for the implementation of both informal and formal training and record documentation and outcomes of training.
MEDICAL SERVICES:
Responsible for medication administration as assigned, adhering to all agency policy and procedures.
GENERAL RESPONSIBILITIES:
Responsible for daily attendance, supervision and transportation of individuals served. Provide input into the individual`s physical, mental, and financial well-being to the individual`s case manager, supervisor, and services coordinator in writing or verbally of the overall quality of the individual`s services being provided. Provide services to include assistance in feeding, bathing, dressing, toileting, and transporting. Assist in planning, preparing and serving meals in accordance with the dietitians menu`s and specific individual dietary and nutritional needs. Maintain a clean, safe area for the individuals served. Maintain all training supplies, equipment and work / living areas in safe, clean working order. Perform all other duties as deemed necessary.
