VICE-PRESIDENT CARE COORDINATION ? Mercy Health System (MHS) of Southeastern Pennsylvania is seeking a nurse executive for the role of system Vice President for Care Coordination.? This position is based in our corporate offices located in Conshohocken, PA.? The Vice President will provide strategic oversight for MHS`s Care Coordination processes within Mercy Physician Network, Acute Care and Post-Acute Care Patient Care Services.?? This individual will partner with organizational stakeholders across MHS to embed the care processes necessary to ensure the patient care experience and outcomes meet agreed upon measures of success.. ? The Vice President of Care Coordination will have the following major accountabilities: Developing of the MHS vision and strategic planning & implementation strategies for Care Coordination, Palliative Care, Clinical Education and Patient Satisfaction. Establishing effective tracking and reporting mechanisms to ensure the meaningful feedback necessary to drive success Ensuring that MHS Clinical Education is aligned with MHS Care Coordination and Patient Satisfaction goals Instituting structure and processes that drive standardization of practices and policies across MHS in order to continually drive equality and efficiency ? Candidates should possess the following qualifications: Bachelor of Science in Nursing and a Masters Degree in a related field Minimum of 15 years progressive care & leadership experience with a large health care organization or insurer; Care Transition experience is required Six years experience preferred in a CNO role, preferably in a complex matrix organization Knowledge of quality performance metrics and processes and Patient Centered Medical Home criteria Demonstrated facilitation and interpersonal skills, and ability to facilitate large scale change Skills in statistical analysis; Lean/Six Sigma black belt preferred ?
