Description: Knowledge, Experience & Professional Credentials Required: Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree. Pharm D preferred. Currently licensed to practice pharmacy in the state of Kansas. Health System pharmacy experience preferred. Interpret medication orders and accurately transcribe to the computerized patient medication profile. Effectively use the electronic medical record and medication reference material to evaluate medication therapy regimens for contraindications, medication interactions, allergies and appropriateness of medication and dose. Perform pharmacokinetic consults, medication dosing protocols, therapeutic substitutions, warfarin patient education, home medication reconciliation reviews and other services as needed.
Effectively and appropriately communicate cost-effective, evidence based medication therapy to the medical staff, nursing, and other medical center associates. Effectively and appropriately communicate with patients and their families about medication therapy used at home and in the medical center. Effectively document interventional and educational activities in the electronic medical record.
Be active in the development and improvement of policies, procedures and protocols that promote evidence based, cost-effective medication therapy. Ensure the accurate and timely compounding and dispensing of medications. Effectively supervise and coordinate the activities of support personnel. In all activities promote a positive work environment and health care image.
Serving a population of 130,000 in a region approximately the size of the state of West Virginia, the Heart Institute provides heart surgery for the western half of the state. Additionally the 140+-bed facility provides medical, surgical and pediatric care along with cardiac, neonatal and intensive care units; cancer, joint care, diagnostic imaging and eye surgery center; skilled care, home health, and outpatient behavioral health and rehabilitation. A total of 6,651 admission, 97,730 outpatient procedures and 6,871 surgeries were documented last year, while the medical group's specialty and rural health clinics accounted for more than 90,000 office visits. This hospital has a new Center for Health Improvement, incorporating hospital-based fitness and rehabilitation programs with occupational medicine and independent physician clinics.
The hospital maintains a local market share of nearly 90%; with total primary/secondary/tertiary service are at 25%. Patient utilization figures demonstrate approximately one-half of all hospitalizations in the region are maintained in the small rural hospitals surrounded by this facility. Administering perhaps the largest Critical Access hospital network in the country, the hospital earned the 1997 National Rural Health Association Outstanding Program Award for its EACH/RPCH and Telemedicine programs. Under the auspices of the High Plains Medical Information Network, the hospital also provides information technology to 39 rural hospitals across the state together with bio-medical engineering and mobile diagnostics. Centralized laboratory services are available through a joint-venture partnership with a national lab firm.
Sub-specialty clinics in areas as diverse as rheumatology and pediatric cardiology are provided via interactive video through an affiliation with the University of Kansas Medical Center, supplementing the primary care and specialty services of the 74 members of the active medical staff. More than one thousand associates staff the medical center and physician clinics, accredited by the Joint Commission on Accreditation of Healthcare Organizations as well as the American College of Radiology and American College of Surgeons. In each of the last two years, this facility has been nationally recognized by the Press/Ganey organization for outstanding achievement in patient satisfaction, with members of the customer satisfaction and quality departments presenting at their annual leadership conference.
