JOB SUMMARY
The Physical Therapist (PT) is responsible for evaluation, planning, directing and administering physical therapy treatment as prescribed by a licensed physician. The PT administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. The PT assists patients to reach their maximum performance and level of functioning. The staff therapist coordinates, delegates and supervises responsibilities assigned to supportive staff. The staff therapist participates in operational aspects of the department and performance and quality improvement activities within the department. The PT provides input in formulating budget, assists in evaluating department performance versus budget controls and takes appropriate action to remain in budget guidelines. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

SCOPE
Work to maintain given department and profession specific efficiency standard set for home care services. Maintain quality interdisciplinary communication with the home care department as well as other departments within Mercy Health System and outside providers. Assist home care leadership on ongoing assessment of equipment and department needs.

JOB RESPONSIBILITIES
1 Provides direct patient care in a personalized and compassionate manner utilizing the Physical Therapy Process:
A. Successfully demonstrates proficiency of basic skills, equipment, and procedures, including unit specific skills.
B. Follows physician's orders and plan of care.
C. Uses the therapy process and critical thinking skills to plan care with realistic and measurable goals.
D. Provides physical therapy care on weekends when assigned.
F. Reports patient condition/activity to Home Care Director, Clinical Coordinator and team members when appropriate.
G. Supervises activities of physical therapy assistants and/or home health aides if required.
H. Maintains dependable transportation and auto insurance.

2 Documents in a manner which is complete, accurate and legible in all designated areas:
A. Completes OASIS Assessments accurately when required.
B. Assesses and documents patient condition per department policy and as changes occur.
C. Completes discharge summary within 48 hours.
D. Completes documentation according to policy.
E. Notifies nursing of medication changes and documents communication.
F. Completes charting appropriately.
G. Charting reflects implementation of plan of care.
H. Patient education and response is documented.
I. Physician orders are noted prior to home visit and care reflects current orders.
J. Initiates and updates the plan of care per policy.
K. Pain assessment and follow up is performed and documented per policy.
M. Assists in training and orientation of nursing students and new employees.
N. Documents patient assessments and interventions at point of care on the laptop computer and transfers the information to the server twice daily.
O. Completes process (order, OASIS, discharge information in profile, notification of team members, and summary) within 48 hours of discharge.
P. Documents aide supervision every 14 days when assigned.

3 Communication skills as evidenced by:
A. Communicates verbally and non-verbally in a concise, effective, and appropriate manner to colleagues, physicians, patients, and family/significant others:
- Hand-off patient reports to colleagues are complete, concise, and accurate.
- Communicates with dignity and respect to colleagues, physicians, patients and visitors.
- Reports changes in patient condition to Physician and Clinical Coordinator as appropriate.
- Calls physicians as needed and has all required information available.
- Communicates with insurance case managers and requests visit authorization as needed.
- Notifies patients of anticipated visit schedule.
B. Displays a positive attitude.
C. Delegation: makes safe and appropriate requests of team and ancillary personnel and role models effective communication and behavior for team members.
D. Collaboration/Coordination:
- Ensures patient needs are met and adequate supplies available for upcoming visits.
- Demonstrates cooperation and support as a team member.
- Submits anticipated visits to scheduler weekly.
- Coordinates care with ancillary services to schedule visits to facilitate completion and patient satisfaction.
E. Education of patient and staff:
- Acts as a change agent to facilitate new processes.
- Evaluates knowledge and provides verbal and written instruction.
F. Answers all pages and telephones promptly and professionally. Is able to answer questions or direct caller to appropriate personnel.

4 Actively supports Performance Improvement initiatives by following home health policies, procedures, and programs; provides ongoing peer education (i.e., Customer Satisfaction, Physician Satisfaction, Co-Worker Satisfaction, and Process Improvement):
A. Reports occurrences and events to Director, Clinical Coordinator and/or Nurse Case Manager.
B. Reports allergic/adverse medication reactions to Quality Management Coordinator, Clinical Coordinator and/or Nurse Case Manager.
C. Adheres to Infection Control Policies.
D. Adheres to Employee Health Policies.
E. Participates in departmental and organizational process improvement activities.
F. Performs record reviews and chart audits at least quarterly and as assigned.
G. Understands and complies with organizational policies, procedures, Joint Commission standards, Arkansas Department of Health, applicable laws and regulations.

5 Demonstrates behavior that is supportive of cost effective patient care:
A. Sets priorities and utilizes time efficiently in order to complete work on time with least mileage.
B. Ensures supplies used are properly charged and utilized.
C. Maintains appropriate inventory of nursing supplies in car to decrease cost.
D. Demonstrates work habits and demeanor which contributes to the efficient functioning of the department and duties as requested or required. Completes other duties within the department as assigned.
- Is responsive to staffing needs of the department.

6 Enhances professional growth and development:
A. Attends education programs, mandatory inservices, workshops, and unit meetings.
B. Seeks out and utilizes education from professional journals and other accepted resources.
C. Incorporates knowledge obtained into practice patterns.

7 Provides safe environment of care for patients, visitors, and staff:
A. Identifies safety, infection control, and environmental hazards at work and/or the patient home and resolves and/or reports promptly.
B. Demonstrates current knowledge of role in:
- Disaster Plan including documentation
- Fire Safety
- Standard Precautions
- Hazardous Material Identification
- Electrical Safety

8 Provides age specific knowledge and skills where applicable. Works effectively with various cultures and ethnic groups.
A. Uses appropriate tools and resources for age, cultural, or ethnic needs
B. Care provided using age-appropriate modification of procedure and communication.

Job Requirements:
QUALIFICATIONS MINIMUM REQUIRED

Education:
Graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy.

Licensure:
Current Arkansas Physical Therapy license.

Certification/Registration:
Basic CPR certification.
Valid Driver's License.

Other skills & knowledge:

Ability to read and communicate effectively in English.

QUALIFICATIONS PREFERRED
1 year of clinical experience preferred.
Home health experience preferred.

Other skills & knowledge:
(skills, knowledge, abilities)
Strong written / verbal skills and basic computer skills preferred.
Additional languages preferred.
