Will work primarily in home health; however, will involve work in other areas in the continuum of care, including acute, sub acute, nursing home, and possibly some out patient. Must be able to assess patient's condition by evaluating their mental, emotional, and physical status related to their impairments. Will develop and implement a therapeutic plan of care for the patient using the most appropriate method of intervention and frequency while considering the sensorimotor, cognitive, ADL's, educational, recreational, and social activities designed to help the patient develop or regain physical or mental function or adjustment to the impairment. Sets goals with patient and/or family. Will interpret physician's orders, make initial evaluation for OT services, and develop or modify the care plan. Must be able to communicate professionally with patient and family members, as well as the Home Health Interdisciplinary Team. Reliable transportation required, with current driver's license and agency required liability insurance. Must have current occupational therapy license from the State of Nebraska.
