Hospital Based Physical Therapist - PT

Our client needs an outstanding Physical Therapist for their flagship hospital. This facility is a Primary Stroke Center and was recently named one of Americas Best Hospitals by U.S. News & World Report with designations in Heart Care and Heart Surgery, Obstetrics and Gynecology, Kidney Disease and Urology. The hospital is also recognized as a Magnet facility by the American Nurses Credentialing Center, the highest honor a hospital can earn for its nursing care and practices.

Physical Therapist needed for hospital treatment of patients with brain injury, stroke, cardiac disease/surgery, spinal cord injury.

JOB SUMMARY: This position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served. Service Excellence is demonstrated by consistently exhibiting the Service Standards in the performance of this job.

ESSENTIAL FUNCTIONS:
1. Evaluates patients functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals.
A. Completes comprehensive evaluation within the established time frame set by the department.
B. Identifies and prioritizes key limiting factors.
C. Establishes treatment goals that are functional, measurable, patient related and reflect the key limiting factors.
D. Collaborates with patient and family/caregiver when setting goals.
E. Initiates discharge planning.
F. Recommends additions to or modifications of referring orders.

2. Implements individualized treatment plan with consideration to the patients physical, social,
spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner.
A. Assesses ongoing effectiveness of treatment and modifies plan of care as indicated.
B. Involves patient and family/caregiver in the treatment and decision making process.
C. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.

3. Documents according to standards set by the department and regulatory agencies.
A. Documents legibly.
B. Completes documentation within the established time frames of the department.
C. Documents innovative and individualized treatment techniques/plan.
D. Documentation reflects comprehensive assessment of patients progress or lack of progress.
E. Treatment goals are reassessed according to department policies.

Requires the ability to read, write, speak and understand English. Must demonstrate effective verbal and non-verbal communication with internal and external customers. Requires good teaching skills. Ability to organize patient assessment data and document the evaluation/treatment accurately and completely within the time constraints of a daily patient schedule. Must be able to problem solve effectively with patients, families, staff and physicians.

Must have 3 years experience either in acute medical hospital or acute rehabilitation center.
Must have graduated from an APTA accredited school of Physical Therapy.
Must possess a current Physical Therapy license from the state of Arizona, or have the ability to obtain.
