Job Summary:

Coordinates Social Services for the Home Care department; initiates and maintains contacts appropriate to these services, and support the Hospice philosophy.

Job Description:

Essential Functions:
  • Acquires current information of psychosocial research through library literature searches, attending workshops and independent study, as observed by director, and team supervisor.
  • Assesses risk factors of bereavement clientele, and completion of risk factor specific assessment during the hospice assessments.
  • Assists nursing staff in patient identification and referral profess for medical social work intervention. Problem identification of psychosocial issues through social work diagnosis and funding source guidelines (Title 19 Medicare, Champus, private insurance), and care conferences with staff and supervisors.
  • Completes and submits progress notes and discharge summaries to physician through use of appropriate forms and problem resolution specifics within five (5) working days of discharge, as observed in chart review.
  • Completes psychosocial assessment and assesses specific treatment plans for appropriately referred patients by use of social work diagnosis, problem solving, and goal setting approved by current medical social work intervention guidelines. Doctor orders for medical social work will be secured prior to home visits and assessment. Schedules visit within two days of referral unless orders specify otherwise.
  • Completes psychosocial assessment and financial needs assessment, when appropriate, through home visits with family and patient using social work diagnosis and problem solving intervention within three (3) days of admission; Hospice consultation and admitting nurse within 48 hours of admission as observed through chart review (admission may take one to two visits, plan to see on consecutive days to complete assessment).
  • Complies with policies on attendance, punctuality, work time, meals, and break periods. Contacts all families participating in Hospice to introduce social work service and schedule assessment visit time within two working days of admission, as observed in chart review.
  • Contributes current knowledge of psychosocial factors as they impact on patient care for staff and community.
  • Coordinates bereavement services for Hospice families in assigned rural areas who desire to participate in bereavement follow-up.

    Additional Requirements:

    Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
