Duties:
Provide and document evaluation and treatment to individual and groups of patients through planning, implementing, and collaborating with patient and caregivers. Provide care for patients according to the standards of care specific to age and the individual’s needs.
Qualifications:
Doctorate of Physical Therapy or Master’s degree in Physical Therapy (or grandfathered in with a bachelor’s degree as per requirements, to be reviewed by director). Licensure or proof of eligibility for licensing by the State of Wisconsin. CPR certification. Completion of appropriate clinical affiliations. APTA membership preferred. Flexible schedule to change hours and/or days as necessary. Ability to work autonomously and deal with a variety of people. Ability to read and follow written/verbal protocol. Demonstrate effective communication and organizational skills.
