Current BLS or CPR certification is required
Current WY RN License is required

ROLE SUMMARY Responsible for managing the overall operations of the parent home health agency, home care branches and other home based services as applicable. Responsibilities include fiscal, clinical and operations management as well as marketing, planning and implementing home care services that meet market needs. This individual manages all agency staff in their performance of service for clients. Facilitates communication inter and intra-departmentally as well as in the community. CORE RESPONSIBILITIES 1. Plans, implements and continually evaluates the overall services of the home health agency. Develops and updates department goals and objectives in collaboration with department employees. Assures standards of practice are met by all disciplines of direct care staff. Monitors overall care of patients and establishes agency goals for service delivery. Delivers a high level of customer service to all internal and external customers and ensures that all staff deliver the same level of service. Assures a timely and appropriate response to customer issues. 2. Ensures the provision of cost effective, high quality home care services. Analyzes financial outcomes to operate the agency within budgetary projections, and appropriately manages variables to assure cost effective operations. Maintains staffing practices within established benchmarks to ensure cost efficient, high quality service delivery. Participates in facility operations planning and budget process. Determines staffing, visit/hour volumes, space, equipment, other expenses and revenue to be used as the basis for the agency's annual budget preparation. 3. Works with the CQI Coordinator to implement and manage the Quality Improvement program to continuously assess and improve the performance of home care services provided in the community. Implements and manages the HCS compliance plan. Ensures the agency's practices and documentation meet local, state, Medicare, JCAHO, insurance and payer requirements as well as agency policy. Reviews policies/procedures and establishes community variations with the input from appropriate staff. 4. Implements and updates a business development/marketing plan to promote home care services to referral sources (including physicians). Actively markets home care to various constituencies within the community. Plans and implements new business programs to meet community and client care needs in a changing environment. Maintains an active role in community and public relations activities. Works with Communications Department to develop accurate, effective brochures, news releases, advertisements. 5. Manages the hiring, skills development and education, evaluation, counseling, and end-of-employment process for all staff. Communicates consistently and frequently with the Administrative Director, Continuum of Care about all issues and developments appropriately. Demonstrates leadership competencies through communication and relationship building, developing self and others, teamwork, and fostering commitment and understanding of the CRMC values. 6. Facilitates communication within and outside the department. Plans and leads various agency activities and meetings including professional advisory committee meetings, staff meetings, business development activities, and customer service programs. Acts as a liaison with surveyors in conjunction with the home care Leadership Team. 7. Attends all mandatory education and completes 20 continuing education hours annually SKILLS, KNOWLEDGE, AND ABILITIES · Communicates effectively both verbally and in writing with other team members to assure clear and efficient dissemination of information. · Solid knowledge of nursing (especially community health nursing), community resources, CQI management. · Demonstrated knowledge of the budgeting process. · Must be able to assess a patient's condition, formulate a Plan of Care, select appropriate interventions, evaluate patient's response to care / treatment, and to explain / teach patients about their condition. Problem-solving ability is essential and required daily in coordinating patient care and office functions. MINIMUM REQUIREMENTS · BSN required, MSN preferred. · RN licensure in the State of Wyoming. · Current Wyoming driver's license and proof of automobile insurance. · Current CPR certification. · Five years clinical experience in acute care or home care is required. Two years of management/supervisory experience in home health or health related field is required with preference given to candidates possessing experience in operations and fiscal management. Must possess well developed leadership and interpersonal skills.
