Position Summary:
Provides clinical care coordination of assigned patients monitors patient progress coordinates with physician to ensure patients are treated at appropriate level of care provides clinical reviews to insurance and provides transitional planning for post acute care. Serves as a resource on case management issues to physicians and nursing staff.
Key Responsibilities/Essential Functions:
Other duties may be assigned. Regular and reliable attendance is an essential function of the job.
- Primary functions of this advanced practice nursing role in Case Management Department are: 1) clinical care coordination 2) utilization management and 3) transitional planning. Responsibilities which demonstrate these functions include but are not limited to:
- Ensuring an interdisciplinary plan of care is established for patients
- Monitoring patients' progress toward outcomes
- Collaborating with physician and communicating effectively care recommendations
- Supporting staff nurse in integrating and analyzing clinical findings
- Ensuring patients receive appropriate care according to clinical needs
- Coordinating with physician to assure intensity of service and severity of illness supports level of care
- Providing clinical reviews to third party payors to secure reimbursement
- Assessing clinical documentation and supporting the documentation coding team
- Ensuring a safe and clinically appropriate discharge plan
- Collaborating with multidisciplinary team members to coordinate care
- Documenting assessment plan and interventions in medical charts
- Maintaining statistical information for reports as required
- Collaborating closely with Social Work Case Manager team member.
Qualifications:
Education and/or Experience
- RN BSN with hospital experience.
- Clinically knowledgeable of nursing care in area assigned

- Registered Nurse (RN) in state of Kansas

- Patient age range is newborn to elderly, of all socioeconomic levels and from a wide variety of cultural/ethnic backgrounds. Patients have a wide range of medical conditions in this regional tertiary care medical center.
