Job Description:
Position Summary: The function of the Case Manager is to assist the interdisciplinary team in facilitating the delivery of services to patients/families; manage the financial resources of the persons served by acting as liaison with the payer and insuring cost of effectiveness of treatment; facilitate and coordinate the discharge planning process; serve as the patient and family advocate to coordinate the direct effective communication regarding the persons served. The Case Manager meets with and makes him/herself available to all assigned persons served regarding identifying issues concerns or needs they may wish to express in order to facilitate quick problem resolution and a satisfying treatment experience. The Case Manager has immediate access to all levels of hospital management for problem resolution including Medical Director Medical Staff Administration and Department Leaders. Essential Functions:  Other duties may be assigned. Regular and reliable attendance is an essential function of the job.Coordinated the provision of care for each person served to ensure achievement of outcomes by the following:-          Completion of assessment within 48 hours of admission -          Participating in team meetings and family conferences -          Coordinating the individualized physical functional and psychosocial treatment program with person served-          family physicians nurses therapists and others directly involved -          Assist in coordination of home evaluations and/or day passes with appropriate team members -          Coordinates the person's served discharge plan to include referrals equipment and appropriate resources -          Facilitates orientation of the person served that is appropriate to the services and the outcomes predicted. Interaction with the persons served team of each person served and other stakeholders by: -          Providing information and education in the area of Living Wills/Advance Directives Durable Power of Attorney  -          Conservatorship and Guardianship -          Assisting persons served/families with Medicaid Veterans and Social Security Disability/Supplemental Income -          applications -          Maintains awareness of financial constraints/contracts to maximize resources to the patient -          Providing supportive counseling to persons served and families if appropriately licensed -          Informing appropriate team members of psychosocial issues as indicated -          Ensuring appropriate family members and significant others are assessed involved in the treatment planning -          and encouraged to participate in family educational opportunities -          Communicating pertinent information and/or issues to appropriate disciplines when necessary -          Communication with third party payers as appropriate. Be an active department employee by: -          Supports and communicates the mission philosophy and values of Via Christi in daily work performances and -          interactions with staff patients and visitors of the RehabilitationCenter -          Accepting responsibility for one-to-one communication and responding non-defensively and/or assertively -          when interpersonal problems or conflict occur -          Assuring that documentation is complete appropriate and recorded in a timely manner -          Attending scheduled meetings -          Assisting with program evaluation activities to meeting JCAHO and CARF standards -          Identifying quality assurance issues recommending an effective action plan and opportunities to improve patient -          care and services -          Assisting with patient care satisfaction -          Recommending strategies to improve length of stay efficiency and resource consumption within the case -          managed patient population -          Acting as a positive team member clinical role model and assisting staff with positive problem solving -          Attending local/regional continuing education activities which relate to clinical scope of practice for age greater -          than two (2) years. This will ensure knowledge of the rehabilitation programs being provided and enhance -          professional growth of self and department staff. Shares information learned by attending educational programs -          with other staff members formally and informally -          Participating in yearly Personal In-service Education process.

Qualifications:
