Position Summary This position practices in collaboration with the physician(s) to provide comprehensive management and follow-up care for advanced heart failure, high-risk heart failure patients, and management of complex congenital heart disease patients who are at risk to develop heart failure. This position functions in collaboration with cardiology physicians to provide advanced assessment, treatment, and coordination of care for heart failure patients and is responsible for appropriately triaging patients based on vitals, managing treatment plans and medications. Care may include physical exams, diagnostic studies, prescribing medications/treatments, consulting on patient management plans, and making referrals. The incumbent must have effective communications skills with patients, coworkers, and physicians. This position spends the majority of time standing (up to 60% of work time) with frequent walking of short distances (up to 20% of work time.) Licenses/Certifications Required Arkansas APRN or Physician Assistant, BCLS Specific Educational and Skill Competencies Required AR license as an RN and registration as a nurse practitioner or physician assistant is required. 3-5+ years experience treating complex heart failure patients in an outpatient and/or inpatient setting strongly preferred. Will maintain collaboration agreement with clinic MD as required by the Arkansas State Board.
