The role of the RN - Nurse Navigator reports to the to the Clinic Manager in coordination with the Clinic Physician. This position is responsible for providing quality patient care in accordance with the TAVR program. This position is responsible for overseeing all aspects of conducting clinical trials. This position is responsible for ensuring strict protocol adherence and study regulations are met, as well as study budgets and timelines. This position performs responsibilities associated with the coordination and application of research study/projects.
Essential Job Responsibilities
Identify patient eligibility for ongoing clinical trials
Coordinate the referral, screening, testing requirements, consultation appointments and hospital-based procedures
Facilitate communication with cardiovascular surgeons and cardiologists, including referring physicians regarding patient eligibility and clinical status
Serve as a point of contact to all patients from diagnosis, treatment, and recovery period
Collaborate with the interdisciplinary care team regarding services to ensure efficient movement of the patient along the continuum of care
Provide professional education to clinical team members in the advanced techniques and care of patients receiving specialized interventions
Facilitate the progression of the valve procedure patient and assist with preparing the patient and family through providing necessary education
Provide follow-up care per pre-determined clinic intervals, documents patient status and communicates ongoing patient condition to physicians during follow-up care
Collaborate with an interdisciplinary cardiovascular health care team in planning, implementing and evaluation ongoing care programs and education for the patients, families, care givers and community
Develop and implement program improvement strategies to ensure program optimization
Provide community physician outreach and education using nursing experience and networking in conjunction with physician leadership to build referral program within expanding markets
Partner with leadership to develop and implement policies and procedures affecting the valvular program
Ensure compliance with all regulatory requirements, adhere to National Patient Safety goals and comply with facility policies, protocols and procedures
Education: Must be a graduate of an accredited nursing program
Licensure and Certifications: Registered Nurse license, required. CPR, required. Valid drivers license and auto insurance, required.
Experience: Minimum 5 years with 2 years cardiology experience, required. Previous experience with TAVR patients or TAVR program, preferred.
