Position Summary The incumbent is responsible for providing continuity of care to assigned patients from inpatient to home, LTAC, rehabilitation or skilled nursing facility. Responsibilities include, but are not limited to participating in the management of the symptoms of chronic illnesses, educating patients and families regarding chronic illness treatment and prevention, as well as communicating the goals of: care, advanced directives, prognoses, lab and other clinical test results with patients and families. The incumbent is responsible for maintaining awareness of the patients health status including IADLs, AODLs, depression, cognition, non-licensed caregiver skills and medical equipment needs as well as performing ongoing medication reconciliation. He/she develops personalized urgent and emergency care plans with patients and families. The incumbent collaborates and coordinates care with other health care providers across the spectrum of care. He/she attends primary care visits and communicates findings/developments with responsible providers. He/she identifies the patients medical home and follow up plans. This position functions in offices, homes, and clinical settings, with exposure to communicable disease and body fluids. The majority of time is spent standing and walking of short distances (up to 50% of work time) with frequent sitting when driving between appointments (up to 40% of work time.) Occasional lifting and carrying of objects weighing up to 25 lb. (up to 10% of work time.) Frequent local travel and use of a personal vehicle is required. Licenses/Certifications Required Arkansas RN License, AR Drivers License & Auto Liability Insurance Specific Educational and Skill Competencies Required Must be licensed as an RN in AR or a participating compact state, have access to a personal vehicle, an unencumbered drivers license and auto liability insurance. Honed interpersonal skills are essential (both verbal and written) to effectively interact with all age and socioeconomic groups. Bilingual skills (English/Spanish) are preferred. Must be familiar with Microsoft office products and have excellent time-management and record-keeping/documentation skills.
