POSITION SUMMARY The successful candidate is responsible for the coordination of Practice Transformation Initiatives within a primary care location. Responsibilities include: providing patients with timely and accurate reminders related to preventative care services; pre-visit planning for high-risk patients to include gaps in care such as immunizations and preventive screening exams; provider notification of gaps in care for the individual patient; maintenance of accurate documentation of referrals, specialist visits, and ED/Hospital stays; providing pre-prepared, chronic-disease, educational materials to primary care patients; active participation within the transformation team; review of monthly dashboards; documentation of interventions; and additional duties as assigned within the scope of the Care-Coordinators practice. He / She may also participate in the analysis of patient data and may assist with the application of risk stratification screening tools to meet program requirements. The incumbent possesses knowledge of preventative care measures. He/she may assist the team with the identification of social, clinical, and economic barriers in order to plan for individual practice transformation activities. This position functions primarily in clinical settings, with exposure to communicable disease and body fluids. The majority of time is spent standing and walking (up to 60% of work time) with frequent sitting (up to 40% of work time.) This position is frequently required to lift and carry objects weighing up to 10 lb. (up to 20% of work time.) LICENSES/CERTIFICATIONS REQUIREDIf AR LPN, must maintain license in good standingSPECIFIC EDUCATIONAL AND SKILL COMPETENCIES REQUIRED FOR THIS POSITIONLPN (or Medical Assistant with 2 years experience in primary care setting may be considered). Prior experience within a primary care setting is essential. Exceptional interpersonal skills to effectively interact with all age and socioeconomic levels are required. Good written and verbal communication skills a must. Basic computer skills required. Knowledge of NextGen, Greenway, Intellichart, Allscripts, Cerner and PICIS software and/or experience in PCMH and other practice transformation models a plus.
