Job Description:
Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Demonstrates responsibility and accountability in performance of psychosocial assessments and implementation of patient care in relation to dismissal needs and follow up care with all age populations. Advocates for patient/family/significant other and interdisciplinary team approach to dismissal care planning. Provides patient and family education, support and advocacy. Serves as resource and liaison connecting patients and families with information and resources.
Mandated reporting of abuse and neglect
Continuum of care utilization management
Community resourcing
Problem solving/judgment (Finds cases with high social risk. Intervenes to resolve system problems which delay delivery of patient care.)
Job Requirements:
Preferred knowledge and skills:
Demonstrate a knowledge of health care reimbursement
Possess a working knowledge of medical necessity criteria
Possess a working knowledge of performance improvement
Possess a working knowledge of mental health illness and treatment modules in the geriatric patient population
Minimum education and experience required:
Bachelors degree in Social Work
Candidates with three to five years recent clinical acute care experience given first consideraton
Prefer case management experience and mental health experience.
Licensure or registration required:
Current licensure with the Kansas Behavioral Sciences Regulatory Board
