General Summary:
Under general supervision, performs secondary triage screening to determine appropriate avenue for care of patients.  Ensures care of patient by appropriate provider. Monitors access to care. Educates patients and families in home care techniques and the responsibilities of managed care.


Duties and Responsibilities:
The following are essential job accountabilities:

1 Obtains history and gathers triage information to determine appropriate avenue for care.

2 Verifies patient insurance eligibility and identifies primary care physician.

3 Relays triage information and facilitiates communication between the provider and the patient concerning patient disposition.

4 Assists physicians with patient care and disposition as needed.

5 Assists in the management of patient utilization of health care system and provides patient education relative to health care needs, use of community resources, and responsibilities of membership in managed care.

6 Provides telephone advice according to established guidelines and protocols. Monitors compliance and evaluates effectiveness of home treatment instructions.

7 Develops telephone triage guidelines as needed.

8 Performs nursing duties as required.

9 Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.

10 Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.

11 Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

12 Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.


The following are nonessential job accountabilities for ADA purposes:
13 Attends meetings as required.

Performs other related duties as identified.


Qualifications:
Note: Other combinations of formal education, training and experience may be considered.

Minimum Education Associate's Degree

Minimum Experience 3 - 5 years (broad clinical practice)

Req Cert/Registration Registration and current RN license in PA; BCLS certification

Mostly sedentary work
Occasional reaching, stooping, bending, kneeling, crouching


Working Conditions
Contact with patients under wide variety of circumstances
Subject to varying and unpredictable situations
Handles emergency or crisis situations
Occasionally subjected to irregular hours
Occasional pressure due to multiple calls and inquiries
