Job Description
Coordinator – Planned Care Opportunity For York and Adams County offices
About the Position
Under the general supervision of the WMG Administrator and Manager-Performance Improvement, works in a collaborative manner with the Planned Care Leadership Team, coaches, practice managers, physicians and clinical support staff to integrate planned care into operational work flows using process improvement methodologies and identifying “best practice.” Assesses, plans, implements, coordinates, monitors and evaluates process improvement and change management options for spreading planned care across the medical group, using communication and available resources to promote high quality, cost effective outcomes. Develops necessary training in process improvement to ensure adequate support of coaches, providers and staff. Acts as a change agent and a champion for change.
The qualified candidate will be responsible for carrying out the following duties:
- As a member of the Planned Care Leadership Team, uses knowledge of project management and performance improvement tools to assess, plan and implement strategies to operationalize planned care.
- Ensures alignment between WMG and other quality measures, the objectives of the Planned Care Collaborative and the work of the coaches and physician practice leadership teams.
- Works in a collaborative manner with the Planned Care Leadership Team, coaches and practice leadership teams to integrate planned care into physician practices.
- Oversees and coordinates the work of coaches and practice leadership teams in implementing planned care in WMG practices.
- In collaboration with Quality Management, identifies, assesses and plans for educational needs of coaches.
- Ensures adequate resources and training for the provision of coaches to fulfill the stated objectives of the collaborative.
- Develops linkages with external organizations (e.g., RWJF, IPIP, ASQ) to share resources and ideas.
- Plans, organizes, and facilitates monthly meetings of the collaborative, as well as monthly contacts with coaches and Practice Leadership Teams (for WSH and non-WSH practices).
- Provides regular updates to Planned Care Leadership Team, Advisory members (including RWJ AF4Q project manager) and others as required.
Qualifications:
- Bachelor’s Degree
- 5 – 8 years work experience across the spectrum of healthcare delivery and operations environment preferred
- 2 years experience in facilitating improvement teams
- Lean Six Sigma experience (Green or Black belt)
- Experience in Chronic Care Model and Patient Centered Medical Home desirable
