Job Description
GENERAL DESCRIPTION:
Provides professional nursing services to individuals in the home, through a process of nursing assessment, planning, care and evaluation in accordance with physicians orders, the established plan of care, the patients diagnosis, corporate policy and procedures, and the Nurse Practice Act. Carries primary care management functions for assigned patients.
SUPERVISION RECEIVED:
Reports to and receives direction and consultation from Clinical Management.
SUPERVISION EXERCISED:
Provides patient care supervision to Licensed Practical Nurses, Home Health Aides, and Volunteer staff caring for assigned patients.
PERFORMANCE EXPECTATIONS:
As Related to Patient Care
1 Assesses the physical, psychosocial, spiritual, safety and environmental needs of assigned patients on the initial visit and reassesses same on each subsequent visit.
2 Develops a written plan of care in accordance with the patients diagnosis, and family needs, physician orders, and the corporate policies and procedures. Coordinates the development of this plan with other disciplines providing care for patient.
3 Utilizes appropriate disciplines and community resources for optimum patient care.
4 Modifies the written plan of care as necessitated by changes in patients condition.
5 Notifies physician and other care agencies of changes in patients condition and care needs and obtains new orders, as needed.
6 Administers nursing care in accordance with the established plan of care. This may include but is not limited to therapeutic treatment, teaching, counseling and supervising patient and/or caregivers in performing specific procedures.
7 Performs case management functions, when assigned as primary clinician.
8 Participates in patient care conferences with other members of the interdisciplinary team and recommends patients for such conferences. Documents results, when functioning as the patients primary clinician.
9 Documents observations, assessments, interventions of visits, phone calls, and conferences in the patients clinical record on a timely basis and legible manner. Submits documentation according to agency guidelines.
10 Discharges the patient when appropriate, notifying the physician and Manager. Instructs the patient and/or family how to access necessary follow-up.
11 Provides both direct care and telephone consultation on a rotating basis as assigned, twenty-four hours a day as directed.
As Related to Staff
1 Functions as a member of an inter-disciplinary team, encouraging and maintaining open communications at all times.
2 Provides clinical supervision to the home health aides, licensed practical nurses or volunteer staff, for assigned patients and provides input regarding their performance to the appropriate manager.
3 Assists in the orientation of new staff or continuing education of existing staff as requested.
4 Participates in the student nurse program, as delegated.
C Related to Corporate Management
1 Submits patient visit schedule to scheduler and communicates significant changes.
2 Participates in the utilization review/quality assurance program.
3 Functions in a cost effective manner in regards to utilization of time, mileage and supplies.
4 Meets or exceeds established productivity and quality assurance standards.
5 Seeks consultation and direction from management personnel as appropriate.
6 Informs Manager of perceived problem areas related to patient care, corporate management or community relations.
As Related to Community
1 Demonstrates a professional attitude, sound judgment and continuity in interactions with physicians, other health care providers, and community agencies.
2 Represents the VNA at community activities, as delegated.
3 Promotes a positive corporate image.
4 Acquires and utilizes knowledge of community resources.

5 Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.

Department Description:
WellSpan VNA Home Care provides skilled nursing, physical therapy, occupational therapy, speech therapy, home health aid and social work visits to patients who require intermittent health care in their homes. Care is primarily focused on short episodes (6-8 weeks) of recovery after the surgery, the onset of a new diagnosis or the exacerbation of an existing condition, and is ordinarily covered by insurance. Employees work together to provide exceptional, quality care to members of the York and Adams County communities, via direct patient care or support services enabling the care to be provided.
