Job Description
Opportunities available for: Occupational Therapist or Occupational Therapist Assistant (COTA) VNA Home Health *Full-time and part-time available* York, PA
WellSpan VNA Home Cares staff of nurses, aides, therapists and other caregivers works with physicians to provide the services necessary to treat patients in the comfort of their own home.
WellSpan Rehabilitation therapists who work in home health have a diverse caseload, including orthopedic, neurologic, geriatric, lymphedema, chronic pain and postsurgical patients. As part of the team, you will help provide stimulating and challenging care to patients in the home environment.
Our team takes a multidisciplinary approach to patient care, which requires you to work closely and coordinate care with other therapy disciplines and nursing. Caregivers within our integrated health system work hard to provide open communication and smooth handoffs among various levels of care.
Under general supervision, the Physical Therapist provides physical therapy services at an entry to intermediate skills level to individual patients in the home according to the principles and practices of physical therapy and works collaboratively with the interdiciplinary treatment team.

Qualifications
- license and registration as required
- Strong communication and written skills
- Computer skills (Electronic health record is used for documentation)
- Travel required (50-60 miles per day) with mileage reimbursement
