Job Description:
Provides psychosocial evaluation, psychotherapeutic intervention and discharge planning services for patients and/or their families. Provide a liaison between the physicians and the patient and/or family and between the hospital and community agencies.
Job Qualifications:
Master's Degree or equivalent formal training in field of specialization and one (1) year experience with License as Master or Independent Social Worker, or License as Associate or Professional Counselor required.
