ED Case Manager

7p-7a (72 hrs/pay period), every third weekend
SUMMARY:
Facilitates the patient's progress in the emergency episode through levels of care in order to ensure quality while being efficient and cost effective. In collaboration with the patient/family, multidisciplinary team including physicians and payors the ED Case Manager provides ongoing case assessment and monitoring of the patient's clinical, functional, and psychosocial status and maintains coordination of the care experience across the continuum.
REQUIREMENTS:
Bachelor's degree in Nursing (BSN) or a related health care degree or equivalent combination of education and experience. BSN preferred.
ESENTIAL JOB-RELATED EXPERIENCE:
A minimum of three years of acute care nursing experience in area of assignment (Emergency Medicine) is preferred. Knowledge of utilization review or managed care preferred.
