Position Overview:
Provides patient care to patients in a variety of settings including evaluation plan development plan implementation and modification documentation education and communication.
MAJOR RESPONSIBILITIES JOB DUTIES AND PROJECTS:
1. Completes appropriate thorough and ongoing assessment of patients (evaluations assessment tools).
2. Develops individualized patient plan of care (diagnosis specific age appropriate).
3. Implements plan of care and modifies as needed (interventions equipment modalities).
4. Documents patient care and charges in accurate and timely manner.
5. Provides patient care daily productivity standards to be set by clinical area
6. Appropriately delegates treatment responsibilities to therapy assistants and/or rehabilitation technicians.
7. Provides education to patients and their families staff students volunteers and public.
8. Effectively communicates with other team members in order to coordinate patient care.
9. Shares responsibility for area staffing and meeting departmental and productivity goals.
10. Maintains annual competencies.
11. Demonstrates and promotes being a good team member as displayed by flexibility willingness to assist in different departmental service areas and showing respect and cooperation with all departments and individuals.
Education Requirements:
Graduate of a program in physical therapy.
Experience Requirements:
One year experience preferred
LICENSE/CERTIFICATION/ REGISTRATION:
Current Kansas license in Physical Therapy current CPR certification.
