Position Overview:
Under the supervision and direction of the Director of Pharmacy/staff pharmacist fills routine orders for oral IV medication and IV-admixtures. Assists in inventory management of pharmaceutical products and performs assigned function on the computer. Keeps the department equipment IV hoods shelves refrigerator and freezer clean and in compliance with infection control directives.
MAJOR RESPONSIBILITIES
JOB DUTIES AND PROJECTS: IV-ADMIXTURE AND THE IV ROOM:
1. Makes daily rounds to floors to determine patient requirements for the following twenty four hour period return discontinued IV preparations and other pharmacy material.
2. Enters necessary information into the computer for IV preparation post credits for discontinued IVs if appropriate.
3. Accurately prepares large and small volume IVs in a timely manner and ensures that all preparations are correctly labeled and checked by a pharmacist before they are dispensed.
4. Maintains a sufficient frozen product inventory to meet weekly requirements. Ensures that all pre-packaged frozen preparations are correctly prepared and labeled; the necessary information recorded in the frozen product log book; and checked by a pharmacist with necessary signatures before storage.
5. Checks each unit periodically and before daily departure to ensure that all IV preparations are stored under proper conditions.
6. Generates a weekly IV supply purchase order for Materials Management; receives order checks for accuracy and if sufficient inventory exists to meet requirements.
7. Cleans the IV hood with log book documentation twice daily; upon arrival before mixing and at mid-day (after lunch). Records daily freezer and refrigerator temperatures in the log book. Reports any temperature variance to the Director of Pharmacy or Staff Pharmacist.
8. Maintains records of IV hood culture and sensitivity studies and biannual certification contracted through an outside service.
9. Restocks the IV room daily before departure.
10. Maintains an adequate supply of clean cloths for hood cleanings and removes soiled cloths and delivers soiled cloths to the laundry when indicated.
UNIT DOSE DRUG DELIVERY:
11. Generates a computerized pick list accurately fills and rechecks unit-dose cassettes using pick list ensures that cassettes are properly labeled and delivered in a timely manner.
12. Removes discontinued medication from the cassette and return items to pharmacy including medications for patients that have been dismissed for credit.
13. Informs pharmacist of any information or problems encountered when filling rechecking or exchanging cassettes.
14. Ensures that there are no shortages of floor stock narcotics bacteriastatic saline MOM etc. including sufficient supplies and local anesthetics. Follows policies and procedures for order checking and record keeping.
15. Keeps dispensing area counter space on the dispensing island sink area in the main pharmacy and compounding room clean.
INVENTORY MANAGEMENT:
16. Receives all in-coming orders from wholesalers and direct accounts; checks and records dating for out-dates management and places supplies in their appropriate location in the pharmacy.
17. Delivers special orders to other departments and ensures deliveries are completed and signed invoices delivered to accounts payable in a timely manner.
18. Receives controlled substance inventory reconciles the narcotics received with quantities listed on the invoice; reports any discrepancies immediately to the Director of Pharmacy. Enters narcotics received into inventory accurately recording the inventory and date on the DEA form and into the narcotic inventory log book.
19. Returns out-dates or rejected supplies to the appropriate manufacturer or wholesaler completes the necessary forms to ensure timely credit.
20. Charges "surgery sheets" on a daily basis; maintains a sufficient quantity of "Controlled Administration Record Sheets"; prepares the monthly accounting report for floor stock items issued to the various departments and delivers report to accounts payable.
21. Fills and delivers traditional and automated dispensing machine floor stock to various departments O.R. E.D. etc pre-packages non unit-dose medications as needed.
22. Takes a weekly narcotic inventory of all automated dispensing machines in the hospital; reports any discrepancies that cannot be reconciled to the Director of Pharmacy or designee; completes a physical inventory of all items in each automated dispensing machine at least monthly; ensures that a current inventory list is attached to each machine for unit reference.
23. Reconciles daily the automated dispensing machine charge and credit list with the "upload A4" charge and error report to ensure accurate patient charge and credit accounting.
24. Orders and maintains the supplies necessary for the day to day operation of the pharmacy from Materials Management; including forms labels legal pads typewriter ribbons etc.
25. Assists the staff pharmacist in the following activities: keeping bottles vial drawers supplied; maintain an adequate and orderly supply of plastic bags; filing orders and profiles; obtaining pertinent demographic information and recording this in patient and computer profiles; preparing first doses participating in order entry if requested and emptying pneumatic tubes.
