Position Summary:
Assists the patient with personal care light housekeeping and other responsibilities delegated by a Registered Nurse or Therapist.
Essential Functions:
Other duties may be assigned. Regular and reliable attendance is an essential function of the job. Makes telephone contact with patient within 24 hours of assignment to schedule first visit. Writes future visits on calendar in patient's packet Follows the Aide Plan of Care prepared by the Registered Nurse or Therapist. Provides preventive and restorative personal care including: bathing dressing grooming hair care feeding assistance with elimination routine catheter care routine colostomy care and routine care of prosthetic and orthopedic devices. Completes simple dressing changes that do not require the skills of a licensed nurse. Provides assistance with medications which may include: handing the bottle to the patient opening the bottle and handing the patient water or other liquid to take the medication. Provides assistance with activities that are directly supportive of skilled therapy services but do not require the skills of a therapist to be safely and effectively performed. Examples of such activities are: routine maintenance exercises repetitive speech routines or routine care of prosthetic and orthotic devices. May provide incidental services such as light housekeeping: change of linens light cleaning of immediate area meal preparation kitchen cleanup after meal preparation personal launder essential to the patients health care and trash removal. Contacts the Case Manager or Team Leader regarding any concerns or symptoms of unusual nature. Participates in weekly case conferences with interdisciplinary team to communicate Plan of Care concerns and progress. Utilizes supportive equipment and supplies as needed. Works on weekend and/or holidays as needed. Attends VCHH meetings 75% attendance is mandatory. Maintains and expands knowledge base by attending at least 12 hours of continuing education each year. Education and/or Experience:
High school diploma or general education degree (GED); and completed Home Health Aide training program which meets specific requirements of the State of Kansas. Must be certified as a Home Health Aide in the State of Kansas within four months of employment.
Certificates Licenses Registrations:
Successfully complete a 90 hour Home Health Aide course or Nurse Aide course and State of Kansas home Health Aide Certification. Current CPR certification. Kansas Drivers License and automobile insurance.
