Job Summary
The HIM Clerk performs clerical duties in maintaining resident medical records that include compiling sorting filing collating copying locating verifying completing and transporting confidential information in accordance with established procedures/regulations. Major categories of department are filing record completion transcription (electronic signature) information release reception and Master Patient Index. Duties may cross functional lines.

Essential Job Functions
The employee must be able to perform the essential functions of the job with or without reasonable accommodations. Provide at all times the highest level of customer service to the residents of Via Christi Senior Services family members visitors other employees and volunteers. Assure all medical record information is accurate protected and kept confidential. Protect all medical record information from loss defacing or destruction. File and retrieve medical records at the need/request of management. Demonstrate the ability to identify inaccurate documentation and report to the individual. Monitor for record completion. Ensure that incomplete records are returned to nursing services for correction. Prepare a medical record for a resident to be admitted. Assure all information is complete on admission record. Audit and thin charts for facility every three months. Assist nursing staff to retrieve records and to provide all forms as needed on a timely basis. Mail forms to physicians to the extent permitted by regulations. Audit for complete documentation related to the resident's discharge/transfer. Audit for complete medical record of discharge residents. Assume responsibility for compliance with federal state and local regulations.

Knowledge Skills and Abilities
Typing and computer skills; knowledge of medical terminology. Medical records designee course preferred. Must have excellent oral and written communication skills and interact well with people of all ages. Must be well organized and able to manage time efficiently. The HIM Clerk must have excellent communication skills be able to cope with mental and emotional stress related to the position and function independently having flexibility personal integrity and consistency. The HIM Clerk must perform in such a way to build good working habits and relationships among the individuals they work with and supervise demonstrating a positive supportive attitude. Employee must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads.

Education/Certification
High school Graduate or equivalent.
