Momentum is hiring a Registered Nurse (RN) to support individuals with disabilities. As an RN, you assess the peoples supported health problems and needs, develop and implement nursing care plans, maintain medical records, and administer nursing care to our people. Working under the supervision of our Director of Health Services, you will have the unique opportunity to build a rewarding career in healthcare and to develop caring relationships.
Momentum knows that RNs are in high demand. In making your choice about where you will practice your art, consider the following:
- Our facilities are Intermediate Care Facilities (not SNFs) for people with disabilities, with both Habilitative (ICF/DD-H) and Nursing (ICF/DD-N) homes
- Caregiver to people supported ratio does not exceed 1:5 and is sometimes as high as 1:1
- Nursing caseload is 15-30
- Our home sizes are small (6-bed and 15-bed)
- At our long-term care homes, you have the opportunity to build relationships with the people we support and a team of staff committed to service
- Complex case management will keep you engaged and learning
- High-acuity care is limited or prohibited by facility regulations
- Our Immediate Care Facilities provide wound care per licensing requirements ranging from simple wounds only to a maximum of stage I pressure ulcers
- Hospice care is very limited; when provided, it is done collaboratively with professional hospice care providers who come to our homes
Schedule: 4:00 PM to 12:30 AM, Sunday through Thursday, including overtime and on-call as needed
Starting Rate of Pay: $42.00/hour + $3,000 Sign on Bonus*

Essential Responsibilities:
- Promotes a friendly, comfortable, and safe environment
- Respects and promotes the right of individuals
- Provide direct RN assessment and care management of each supported person with a minimum of 1-1.5 hours per week at multiple locations*
- Quarterly nursing exam
- Review and implement feeding plans
- Provide complex care management and vital assessments
- Develop health care plan
- Supervise LVN clinical responsibilities
- Accompany persons supported to medical appointments
- Ensure doctors' orders are carried out
- Performs other duties as assigned to meet department and/or business needs
Case Management RN Responsibilities Include:
- Complete required nursing reports
- Determine LVN assignments and schedules
- Participate in IDT (Inter-Disciplinary Team) meetings
- Work with Pharmacy Consultant for Pharmacy Audits
- Audit MAR (Medication Administration Record) bi-monthly
- Assist monthly in-service training
- Oversee and support monthly cycle medications
- Schedule doctors appointments
- Ensure specialist referrals and appointments are properly scheduled
- Prepare persons supported charts for doctor rounds
- Receive and record doctors' orders
- Assist physician on rounds
RN Qualifications:
- Graduate of a Board of Registered Nursing approved school; required
- Valid/current California RN License; required
- Minimum of one year of full-time clinical experience as an RN; preferred
- Supervisory and management skills; preferred
- Excellent oral and written skills
- Strong critical thinking skills
- Ability to maintain confidentiality
- Communicate and work professionally and cooperatively with others
- Experience working with adults with disabilities and/or developmental disabilities; preferred
- Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
- Ability to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds
- Valid and current proof of legal right to drive in California; preferred; preferred
*Sign on Bonus will be paid in two payments. First payment will be made after 90 days of employment and second payment will be made after 6 months of employment.
