Clinical Anticoagulation Coordinator Pharmacist

Job Location: Burlington, Massachusetts

Duties: 

Lead clinical pharmacy practice in Anticoagulation Management for Lahey Hospital and Medical Center. Provide comprehensive clinical and distributive medication management services and will utilize advanced clinical skills in the provision of evidenced-based, safe, and cost-effective drug therapy to achieve optimal medication therapy outcomes. Initiate, adjust, monitor, and recommend changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments as related to drug monitoring and adverse effect management, review of medication profiles and the patient medical records. Responsible for ensuring the implementation of safe and effective medication therapy management and monitoring adherence to clinic policies and protocols. Serve as an information and educational resource for the pharmacy staff, patients, residents/students, medical staff, nursing staff, and other healthcare professionals within Lahey Hospital. Participate in the development of clinical guidelines/protocols, new programs/services, and facilitate professional development activities in the Anticoagulation clinic. Interpret laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing. Develop drug use evaluations, ADR reporting, clinic development and management, and create of business plans.

Requirements:               

Doctoral degree in Pharmacy (PharmD) (willing to accept foreign education equivalent) plus one (1) year of experience as a Pharmacist or related occupation in clinical pharmacy specializing in Anticoagulation Management or, alternatively, a bachelor's degree in Pharmacy (willing to accept foreign education equivalent) and five (5) years of experience as a Pharmacist or related occupation in clinical pharmacy specializing in Anticoagulation Management. Specific skills/other requirements - Must also possess the following: Demonstrated Expertise ("DE") designing and implementing tailored programs to enhance patient outcomes through appropriate management of anticoagulation therapy; DE developing anticoagulation policies and procedures to ensure adherence to best practices and patient safety standards; DE providing oversight and guidance on outpatient anticoagulation-related policies and procedures to uphold quality standards; DE evaluating and disseminating pertinent drug information and literature resources to patients, fellow healthcare professionals, and peers, ensuring informed decision-making and optimal medication utilization; DE utilizing electronic health record (EHR) systems, Epic and DAWN AC, to monitor drug-related laboratory data, document anticoagulation therapy, as well as summarize and communicate clinical interventions; Experience may be gained during graduate program of study; and Must possess: Massachusetts Pharmacy License.
